Background: Many countries are adopting reforms such as decentralization to improve public systems and service delivery. The regional tier approach, involving cooperation between districts, is expected to have similar benefits. Findings: Decentralization, similar to the regional tier approach, is expected to enhance allocative efficiency, improve technical efficiency through cost consciousness and resource sharing, and enhance service quality. Conclusion: Implementing reforms like the regional tier system can potentially lead to more efficient and equitable health service delivery, but success depends on addressing governance and organizational challenges.
