Summary
Background: Following World Bank and IMF-backed reforms in the 1990s, many low and middle-income countries decentralized their social sectors, including health. Despite some gains, a decline in services has increased inequality. Findings: Policy options for re-centralization include maintaining the status quo, fully recentralizing health functions under the Ministry of Health, recentralizing regional activities, recentralizing under regional referral hospitals, and outsourcing local-level implementation. Conclusion: There is a gap in getting the health of populations to an optimal level and re-centralizing some or all components may be of advantage.
